Case scenario :
A middle-aged insane female was injured in a road traffic accident at PanthaChowk Srinagar Kashmir She was brought to the hospital by police with a lacerated wound on her anterior abdominal wall.There was no other external injury to the body.
She was declared brought dead in the Emergency Room of SMHS Hospital Srinagar, Kashmir.
Being an unclaimed body, it was handed over to the Department of Anatomy.
After full filling local legal formalities, the cadaver was received, embalmed and dissected later on.
Two years later during routine dissection of this cadaver, we found that her right lung was bilobed with an only oblique fissure in it. Fig 1

The Left lung was single lobed with no fissure(Fig 2).

- The overall weight and volume of both lungs were decreased.
We also observed some other associations in this cadaver were.
- There was agenesis of the left kidney
- The third route of the median nerve was not developed.
- However, other reported association like TE fistulas, cardiac, musculoskeletal and diaphragmatic anomalies were not found.
Teaching message :
- Bilateral pulmonary lobar agenesis is a very rare congenital anomaly. The reported incidence is far less than the actual as cases with a severe degree of pulmonary agenesis die before birth.
- Patients with a minor degree of pulmonary lobar agenesis live a normal life to be detected at autopsy.
- Antenatal diagnosis using modern imaging techniques and proper management in perinatal period can reduce neonatal morbidity, mortality and also prevents brain anoxia and its sequelae.
