CNS Examination The Quick revision

This examination is very important in terms of timing it correctly

Start with checking the Cranial nerves

Cranial Nerves

Say Ok now I am going to check your nervous system… I am going to begin with the nerves

Can you please follow my finger with your eyes ( check the 3rd, 4th and 6th nerve. Write the visual aquity

and visual field in the diagnostic tests)

Now can you clench your teeth for me

Now can you smile for me like this

Now close your eyes tightly for me

Now shrug your shoulders for me like this

Now would you please stick out your tongue

Now move it from side to side

Sensory System

Ok now I am going to check your sensations

Would you please close your eyes and tell me whenever I touch you with my finger

Check on 3 places on face, arms and legs

Check for vibration and proprioception only if the case warrants it eg in diabetics

Motor system

Ok now I am going to examine the power in your muscles

Ok please pull my hand up

Now push it down

Can you please place your arms like this

Now please pull my hand in

Now push it out

Now kick my hand out with your leg

Now pull it in

Now push it down with your feet

Now pull it up

Now I am going to check your jerks

Only perform knee, ankle, biceps, and triceps and brachioradialis reflexes

Coordination

Ok now I want you to touch my finger with your finger and then your nose and then repeat it again and

again

Now I want you to move your hands repeatedly like this (supinate and pronate your hands repeatedly)

Gait

Ok now I want you to get up and walk a few steps for me

Wait let me help you … Pull the foot rest out

Ok now stop

Turn around and come back

Now close your eyes and stand like this

Don‟t worry I wont let you fall
